If you hover your cursor over the Contents tab at the top of the page, you can immediately click on the section you want; alternatively you can scroll down the page.

This section reflects the Care Act 2014 and the Care and Support Statutory Guidance (Department of Health and Social Care). In  particular Promoting Wellbeing, Preventing, Delaying or Reducing Needs and Information and Advice are the three main areas which underpin the Act.

    Person Centred Care and Support

The chapters in this section are a combination of those taken from the Care and Support Statutory Guidance (Department of Health and Social Care) and other areas of person centred care.

    Local Services and Teams

This section reflects the local authority’s services and teams which provide person centred care. Some of these may be commissioned services.

This section reflects the Care and Support Statutory Guidance (Department of Health and Social Care), but also other fundamental issues such as information sharing, user involvement – which is key to the ethos of the Care Act – and staff engagement.
